Phoenix Home Care specializes in the care of children and young adults with special health care needs. Our aim is to enrich the quality of life of the people we serve through the Technology Assisted Waiver Program. Our team is comprised of skilled professionals who are ready to meet the unique challenges of special needs children and young adults as well as their families.
The TA Waiver serves individuals who are 21 years old or younger, chronically ill, or medically fragile and dependent upon a ventilator or medical device to compensate for the loss of vital bodily function and require substantial and ongoing daily care by a nurse (comparable to the level of care provided in a hospital setting) or other qualified caregiver under the supervision of a nurse. In Kansas, services are generally authorized through the the State of Kansas (Medicaid). Services can also be funded through private insurance and Private Duty Care services.
To determine if an individual qualifies for the TA waiver, he or she must meet all three of the following eligibility requirements:
- Meet the HCBS Technology Assisted Waiver definition.
- Require one or more of the identified primary medical technologies, and meet the minimum technology score for the specified age group.
- Meet the minimum nursing acuity level of care threshold for the specified age group.
Nursing Services: Medication administration, g-tube feedings, GJ-tubes, tracheostomy care, personal care, injections, blood draws, IV therapy, TPN, mechanical ventilation monitoring, accompanying to physician appointments, testing, and all aspects of daily routines, including personal care and advanced personal care.
