The person requires intermittent skilled health care services provided by a nurse, physical therapist, speech therapist, or occupational therapist. The person is home-bound, meaning he or she has an illness, medical condition, or injury that restricts his or her ability to leave home without the assistance of others, the aid of supportive devices (such as crutches, canes, wheelchairs, or walkers), the use of special transportation, or those instances where leaving the home is medically contraindicated.

Conditions That Generally Meet the Coverage Criteria Include the Following
- New diagnosis
- New type of medication or medication dosage change
- Recent fall
- Recent exacerbation of a previous diagnosis
To qualify for Medicare or Medicaid home health services, your home health agency must be Medicare and/or Medicaid certified. Phoenix Home Care meets this requirement. Home health care services may be covered under Medicare Part A or Part B.
