Comprehensive Disease Management (CDM) is a completely free, proactive approach that guides you or a loved one through the complex journey of living with a serious illness.

CDM, which includes palliative care, provides foundational support for families through emotional, stressful, and difficult times. The goal of CDM is to improve your chronic and/or life-limiting illness and quality of life, while also providing resources for the entire family.

CDM is a valuable service for children and adults experiencing medical complexity, a significant diagnosis, multiple co-morbidities, and other challenging circumstances that require a higher level of care coordination and advocacy.

What services are available through CDM?

Inspired by Florence Nightingale’s legacy of compassion and dignity, we take a relationship-based approach to your complete care needs.

Comprehensive Disease Management is completely free for any patient who qualifies. You do not need to be receiving other Phoenix services to benefit from CDM.

Caregivers of a loved one with a chronic or life-limiting illness often need help finding and utilizing resources. Our team helps patients and their families connect to and qualify for community resources, including Medicare and Medicaid services.

When you’re caring for a loved one with a chronic illness, planning ahead is important. Thorough advance care planning is essential to ensure your loved one is well cared for throughout their journey when living with a life-limiting illness. Completing Durable Power of Attorney (DPOA) paperwork and end-of-life advance directives provides peace of mind for the entire family. Phoenix assists with a variety of advanced care planning strategies, including completion of DPOA papers and disease-specific advance directives.

When our experts establish your chronic disease management plan, they will identify a number of ways to keep your loved one safer as their disease progresses. In turn, you’ll likely reduce the occurrence of avoidable medical expenses such as emergency room visits and hospitalizations.

Who should be referred for Comprehensive Disease Management?

CDM services are effective for patients in the following circumstances:

  • Medical complexity
  • Significant diagnosis
  • New diagnosis
  • Multiple co-morbidities
  • Functional dependency
  • Multiple hospitalizations
  • Several providers of care
  • Psychological and social upheaval
  • Unmanaged pain and symptoms
  • Unexpected changes, including falls, weight loss, change in appetite, increase in somnolence, significant change in condition

Children and young adults (pediatric patients) can benefit from our Comprehensive Disease Management program. CDM for pediatric patients also includes palliative care services.

Each Phoenix client is assigned a nurse navigator and medical social worker for the emotional and educational support families need. However, our nurses do not provide hands-on care. Learn more about additional Phoenix services that can meet your care needs.

CDM includes a palliative care component to help you or a loved one with pain and symptom management.

CDM services are not the same as hospice services. A physician may recommend hospice when a patient no longer wishes to seek aggressive treatment for a life-limiting illness, whereas chronic disease management patients may continue to seek treatment.
