Only a doctor can discharge you from the hospital. But several members of the care team can provide discharge planning for you, including nurses, social workers, case managers, or even family members. Typically, discharge planning is a team effort.

Support. We’ll determine whether you and your loved ones need caregiver training or other support.

Referrals. Your care team will refer you to various services and organizations, including in-home care agencies, assisted living facilities, primary care physicians, and/or specialists.

Scheduling. Discharge planning includes arranging follow-up appointments or tests.
